- Current Form 5
- Parent's Income Tax Return (ITR) and/or Certificate of Tax Exemption from the BIR if parents are unemployed.
- Endorsement letter from the Office of the College Secretary.
Rates: (based on the slasonline application)

First year students without UP Email may file applications through the SLAS Online. They must log in using the email/username and password issued to them by the University to access the UP College Admission (UPCA) Confirmation Portal. Please be reminded that they still secure a UP Email to avail the various financial and learning assistance provided by the University.

The Office of Student Affairs recognizes the unique role of parents as partners in the formation of our students, and their unique and distinguished roles in promoting wellness and family cohesion. In this webinar, we will discover how we can better enhance our relationships with our parents/guardians, and also award the participants and winners of the recently conducted OSA poetry writing contest. Let us engage in a conversation about how we can celebrate and honor our parents.
